When someone moves into permanent care, the home has a duty to undertake various risk assessments, which are used to inform the plan of care. These assessments and care plans should be reviewed regularly, to ensure they remain up-to-date and relevant. Accurate risk assessments and care plans are essential to delivering safe, effective care, which meets all the individual’s needs. Delayed, inaccurate or out of date risk assessments and/or care plans can result in unintentional harm.
Following discharge from hospital, it is essential that the care home carefully reads the Discharge Summary as it may stipulate changes in the level of care needed, or different doses of medication, or new medication etc. Too often, we see that care homes have not read the Discharge Summary, or not implemented the recommendations, causing the resident to have an accident or injury – which could so easily have been avoided.
Moreover, it is quite common to see that care plans are not undertaken immediately, comprehensively or even at all, or else not updated frequently as the resident’s needs change, unfortunately, leading to accident or injury.
If you are at high falls risk and have fallen whilst a resident in a care home, you may be entitled to compensation.
